NEW PATIENT APPOINTMENTS: If you are NEW to our office, please schedule a New Patient Appointment. If you’re under 18 years of age, a parent or guardian is required to be present for your New Patient Appointment. After booking your visit, please complete the initial paperwork attached to your appointment.

INSURANCE: MSM is currently accepting BCBS PPO and AETNA insurance plans. HSA or FSA cards can also be used and the office can provide any receipt/bill as needed.

CANCELLATION POLICY: If you need to cancel or move your appointment, 12 hours of advanced notice is required. Late cancellations and no-shows are subject to a fee equal to the cost of the visit.

Please note, at this time we are NOT accepting New Patient Appointments whose primary insurance is Medicare or New Patient Appointments who are seeking care for injuries sustained in an auto accident.
